Before you arrive

Know what happens next.

Start with a call. The team can tell you what to bring and where to complete approved patient paperwork. Keep medical details for a private conversation or the practice’s secure intake system.

Plan your visit

Before you come in

Call the office about scheduling, records, insurance, or accessibility needs. The team can tell you which forms to complete through its approved patient system.

At the visit

Share your concerns and goals. Your clinician can review your history, examine your mouth, and explain whether more information is needed before discussing a plan.

After the conversation

Ask for a clear explanation of your options, timing, costs, and what happens next. You should have room to consider your choices.

If your plans change

If you need to cancel or reschedule, call during office hours as early as possible, and at least 48 hours ahead. A missed appointment, or one cancelled with less than 48 hours’ notice, carries a fee of $75 per hour scheduled (for established patients, from the second time). For a Monday appointment, cancel by the preceding Thursday. The full policy is in the patient forms.

When are you open?

Monday through Thursday 8 am to 5 pm. Closed Friday through Sunday and on holidays; please call if you are unsure.

Do you take my insurance?

How do I make an appointment?
